Mixing & Matching Multifocal Lenses: Results of a European multicenter project
Purpose: To study visual performance, functional vision, independence of glasses and patient satisfaction in cataract and refractive patients undergoing bilateral multifocal lens (MIOL) implantation with a diffractive MIOL in one eye and a refractive MIOL in the fellow eye.
Setting: Goes Eye Centre, Antwerp, Belgium and the Mix&Match online-project group
Methods: Patients eligible for bilateral MIOL implantation were recruited and received a ReZoom MIOL in their distance-dominant eye and a Tecnis ZM900 MIOL in the fellow eye. Follow-up visits were performed after 1-2 days, 30-60 days, and 120-180 days post surgery. Binocular uncorrected (UCDVA) and best-corrected (BCDVA) distance visual acuity and uncorrected intermediate (UCIVA) and near (UCNVA) were documented. Reading acuity and speed using Radner reading charts, photic phenomena, independence of glasses and patient satisfaction was recorded.
Results: Twelve study centers recruited 152 patients which passed the 6 months follow-up visit. Binocular uncorrected distance, intermediate and near visual acuity were 1.0, 0.6 and 1.0. Mean binocular BCDVA was 1.0. Reading acuity was 0.3 logRAD under mesopic and 0.1 logRad under photopic conditions and patients could read 198 words/minute under photopic and 185 words/min under mesopic conditions. Eighty-nine of the patients needed no glasses at all after 6 months. Halos and glare were reported as mild and in few cases as moderate. All patients would select the same procedure again.
Conclusion: Mixing and matching MIOLs with different optical principles in suitable patients delivers an excellent visual outcome and a very high degree of spectacle independence.
Title: Clinical performance of Tecnis multifocal acrylic lens ZMA00
Purpose: To investigate best-corrected and uncorrected visual acuity, photic phenomena and independence of glasses with the acrylic version of the Tecnis multifocal lens. Optical bench data showed higher MTF-values compared to the silicone model.
Setting: Goes Eye Centre, Antwerp, Belgium
Methods: In a prospective study 20 patients were operated on both eyes (40 eyes). Three month after surgery uncorrected and best-corrected visual acuity at far, intermediate and near distance as well as the incidence of halos and glare were recorded. Spectacle use and patient satisfaction were documented.
Results: Tecnis ZMA00 showed excellent results in uncorrected distance and near visual acuity. Intermediate vision was sufficient for computer work and to operate cell phones. Overall spectacle independence was 92%. Halos occurred in 25% of the patients but only 1 patient was bothered by photic phenomena. Patient satisfaction was rated 9 on a scale from 0 (very unhappy) to 10 (very happy).
Conclusions: Tecnis acrylic multifocal lens ZMA00 provides an at least similar visual performance as the silicone model. Spectacle independence is high and results in a high patient satisfaction.
